Brookside's Care Transitions Program offers patients and families the opportunity to have an additional post acute care option or when care at home isn't the answer after a hospital or skilled nursing stay.  Enjoy  peace of mind in knowing that your loved one is receiving supervised twenty four hour care with all their daily tasks.  Each transition resident will have  their own  apartment, medication management, engaging programs for leisure time, delicious meals, and laundry and housekeeping services to complete  the package. Working closely with families and caregivers, we assess the individual needs of each resident. We assist families to monitor changes in health including vision, hearing, medications, stress or anxiety. Our memory support program focuses on assisting and enabling our residents to enjoy an active and rewarding life.
