Our Facility

Radius HealthCare Center at Southbridge is a 144-bed skilled nursing facility, staffed with healthcare professionals who attend to the variety of needs of our residents.  Family members can be assured that Radius HealthCare Center at Southbridge is the appropriate choice for their loved one.

Radius Southbridge provides a continuum of healthcare services not only to the community of the Southbridge, but to the surrounding towns, such as Sturbridge, Charlton, Dudley, Brimfield, Holland and Wales. 

Our facility is within close proximity to physicians’ offices and is minutes away from Harrington Memorial Hospital and major Worcester hospitals.  You may choose your own physician to provide your care during your stay.  Our Medical Director is a board-certified geriatrician.

At Radius Southbridge, residents can choose from a selection of accommodations.  All rooms are completely furnished, but residents are encouraged to add their own personal touches.

Our Services

Certified by Medicare and Medicaid, we are also proud to be accredited by The Joint Commission (formerly JCAHO). We also accept private funds and private insurances, including all BC/BS products, Evercare, Fallon, and most other insurances.

Radius HealthCare Center at Southbridge accepts admissions 24 hours a day, seven days a week. We care for short-stay post-acute patients as well as long-term care residents. Some of our specialized treatments include intravenous therapy, tube feedings and pain management.

Our subacute short-stay program features 7 day-a-week rehabilitation services, complex nursing care when needed and includes full case management, and complete discharge planning. Our physical therapy gym provides ample room for rehabilitation activities.

Our Rehabilitative Capabilities

Our Rehabilitation Programs at Radius Southbridge use a comprehensive team approach with each specialist contributing to an aggressive treatment plan. The goal for our patients is to help restore maximum function and independence, and to help in maintaining that independence by focusing on skills training needed for their return home.

Our short-term rehabilitation program’s goal is to return residents to their highest level of functioning with the ultimate aim of discharge to home. For that reason, family involvement in the care plan process is encouraged. Whether the stay at Radius Southbridge is considered short or long term, the patient’s emotional well-being enhances successful outcomes.

 Alzheimer's and dementia care is also offered through Candlewood "Care for the Memory Impaired."
